Direct Primary Care (DPC) gives you an ongoing relationship with me, a nurse practitioner who knows your health history, listens to your concerns, and has time to help you make informed decisions.
Instead of paying a separate office fee every time you need care, members pay one predictable monthly membership fee. Visits may be completed in person, virtually, or by phone when clinically appropriate.

You should not have to wait months for an appointment, feel rushed through a visit, or wonder whether anyone is looking at the whole picture. County NP Direct Primary Care is designed to provide accessible, individualized care with more time for questions, education, prevention, and follow-up. Whether you need a wellness visit, help managing a chronic condition, or guidance when you are not feeling well, you have a trusted healthcare professional who already knows you.
Personalized primary care for one adult member.
Individual Direct Primary Care membership for one child enrolled outside a family membership. Includes age-appropriate wellness care, sick visits, medication management, secure parent or guardian communication, priority scheduling, and care coordination.
A parent or legal guardian must complete enrollment and consent requirements. Outside costs—including medications, laboratory testing, imaging, specialist care, and hospital services—are not included.
Designed for two adults enrolling together and sharing one combined monthly membership cost. Each person remains an individual patient with separate appointments, medical records, treatment plans, and confidential communication.
Includes Direct Primary Care membership for two adults and up to three children in the same household. Each additional child may be added for $25 per month.
Every family member receives individualized care with a separate medical record and treatment plan.
Direct Primary Care is a healthcare membership and is not health insurance. The membership fee covers the professional services listed above. Laboratory testing, imaging, medications, emergency care, hospital services, specialist care, and other services provided outside County NP are not included unless specifically stated.
Insurance may still be used for eligible outside services, including laboratory testing, imaging, prescriptions, specialist visits, and hospital care.
