Members: refills and dose adjustments are handled by message between visits. Non-members: a follow-up visit is needed about every 3 months to continue refills. Please request refills a few days before you run out.
Drawn here through Quest. Members: no draw fee. Non-members: $35 draw fee plus lab cost, or bill your insurance through Quest. Labs from another provider are always welcome.
Intake forms and agreements are sent to your patient portal after you book or join. Please complete them at least 2 days before your visit. To share records from another provider, send them through the portal or fax 515-295-0444.
New patients start with a new-patient visit ($99) from our Home page. Memberships: Direct Primary Care from $99/month, HRT Access $75/month, HRT or TRT $150/month, and weight loss from $75/month. Many plans save 10% when prepaid.
